Sendable, booking and document forms, what's the difference?

Bookem includes three different types of forms

  1. Sendable forms

  2. Booking forms

  3. Document forms

Sendable forms

A Sendable form is used to collect detailed client information, such as name, ID number, medical aid details, and address. These forms are typically sent to new clients upon booking their first appointment or to obtain consent for treatments, such as dry needling.

You can find your sendable forms in the main menu under Forms

Booking forms

Clients complete a booking form online to complete the booking by providing basic information, like name and contact details. These forms can be customised for different services to gather service-specific information.

Most of you service booking forms are automatically set to 'default', but you can create your custom booking forms in Settings > Services > Forms

Document forms

Document forms are used to insert a template with questions, fields, images, or body charts into a document or document template for completion. Document forms were formerly called 'Note forms'.

Find and create document forms in Documents > Forms

This is a New patient assessment form for a physiotherapist with a body chart and observation fields.
