Bookem offers three types of customisable online forms: sendable, booking, and document forms (formerly known as note forms). This article focuses on sendable forms, commonly used for new client intake, consent forms, questionnaires, and surveys.
Before diving into sendable forms, let's first make sure you understand how to use fields in Bookem. Skip this part if you've got fields waxed.
You can include two types of fields in a sendable form:

Fields stored only on the form: These are not saved to the client profile but are filled in automatically when the form is submitted. They’re useful for details like indemnity clauses or client signatures.
Fields that merge with the client’s profile: These fields are stored in the client profile and are automatically filled in when the form is submitted. They’re useful for recurring information, such as medical aid details, which can automatically appear on an invoice.

To create a sendable form, follow these steps:
1. Sendable forms are located under Forms in the main menu on the left
2. To create a form, click '+New form'
3. Give the new form a name and click Create
4. Click + Add a Field
5. Select the fields you want to include in the form
Fields linked to the client profile will be listed separately for easy selection
6. Click Save
Your form is ready! You can now send it to clients or integrate it into your booking workflow.
Oops, if you've made an error or want to add or change something on your form, here's how you can edit a sendable form.
Follow these steps:
1. Sendable forms are located under Forms in the main menu on the left
2. Click on the form to open and edit it
3. Click on any field you want to edit, or delete a field by clicking the red 'x' delete button at the top right of the field border
4. Once you are done editing, click 'Save'
