Bookem offers three types of customisable online forms: sendable, booking, and document forms. This article focuses on sendable forms, which are great for new client intake, consent forms, questionnaires and surveys.
Your Bookem account comes with ready - made templates for sendable forms. You can use them as is, or make changes to suit your practice.
Before you start with sendable forms, it’s helpful to understand how fields work in Bookem. Skip this part if you’re already familiar.
There are two types of fields in a sendable form:
Fields stored only on the form These are not saved to the client profile but are included when the form is submitted. Use these for one-off information, such as indemnity clauses or client signatures
Fields linked to the client profile (Client profile fields) These are saved directly to the client’s profile and auto - fill when the form is submitted. Use these for recurring info like contact details or medical aid numbers, which can show on invoices or other forms
To create a sendable form:
Go to Forms in the main menu on the left
Click + New form
Give your form a name and click Create
Click + Add a Field
Choose the fields you want to include
- Fields linked to client profiles are listed separately for easy access
Click Save
Your form is ready to use. You can send it to clients manually or include it as part of your booking workflow.

You can edit a sendable form at any time. To do this:
Go to Forms in the main menu
Open on the form you want to edit
Click on any field to edit it
- To delete a field, click the red X at the top right of the field box
Click Save when you’re done
