How do I get clients to complete forms manually?

In just a few easy steps, you can provide a form to a client for completion either on your device or theirs. This is particularly beneficial for forms that require completion and signature:

  • During a consultation, such as "consent for a specific treatment" forms.

  • For clients who have yet to complete their online intake form prior to their appointment.

  • To allow clients to check and update their details.

You can either send the form to your client or let them fill it in Via QR Code, for when in-office/on-premise

Send the form to your client

  • Go to Forms and select the form you'd like your client to complete

  • Click 'Send' to email or SMS the form to your client

  • You can add more forms by clicking '+Insert personalised form link'

NB. Each personalised form link is unique to that particular person, do not copy and paste from another client.

Via QR Code, for when in-office/on-premise

Forms can be sent directly to devices for completion in your premises - either on the client's own phone or on a tablet you may have at reception.

Follow these steps:

  • Go to Forms select the form you need completed from your list of sendable forms

  • Click on the 3 dots on the right of the form and select 'Fill in'

  • Select the client from the drop-down list

  • A unique QR code will automatically be generated, and this can be scanned by any QR code reader on your client's phone or a tablet you have in reception.

NB: You could potentially expose sensitive client information by handing over your device to a client. To make things safer, you'll be prompted to log out of Bookem first. This way they won't have access to your data on Bookem. Make sure your Bookem password isn't saved in your browser if you do this.
